怎么把孩子的艺术课转到 AFA · How to move your child's art instruction to AFA using Regional Center funding

第一步:您属于哪种情况? · Which situation are you in?

  • A · 孩子已加入自主决定计划(SDP),有 FMS。 最快,按 A 路线五步走。 / Already in SDP with an FMS: follow Route A.
  • B · 孩子在区域中心传统服务里,还没加入 SDP。 需要先转入 SDP,再走 A 路线。 / Traditional services, not in SDP: enroll in SDP first, then Route A.
  • C · 不是区域中心服务对象。 按常规学生报名试课,自费。 / Not a Regional Center consumer: book a regular trial class.

A 路线:已在 SDP 的家庭 · Route A

  1. 联系 AFA 约一次入学沟通。 电话 (626) 466-5161 或网上留资并注明「Regional Center / SDP」。请告诉我们孩子的情况、IPP 里与艺术/社交/专注力/精细动作相关的目标、FMS 名称、服务协调员或独立协调员的联系方式。 / Contact AFA for an intake conversation.
  2. AFA 出书面报价和服务说明(1–2 个工作日),可直接抄送您的 SC / IF。通常每周一节 1.5 小时一对一,画材包含。 / AFA sends a written quote and service description.
  3. 请 SC / IF 把 AFA 的艺术课写进支出计划,预算类目 SDP 服务代码 331「社区融合支持」。已有别家艺术课的,就是把那一行改成 AFA。 / Ask your SC / IF to add AFA to the spending plan under SDP Service Code 331, Community Integration Supports.
  4. FMS 给 AFA 建档。 AFA 已在 Ritz FMS 建档并接受 ACH,用 Ritz 的家庭这步已完成;其他 FMS 我们按要求提供 W-9、营业执照等。 / AFA is already on file with Ritz FMS; other FMS providers on request.
  5. 支出计划批准后开始上课。 AFA 每月向 FMS 开票,已批准的课程家长不付费。从联系到第一节课通常几周,取决于 SC 批支出计划的速度。 / Start once the spending plan is approved; AFA invoices the FMS monthly.

B 路线:还没加入 SDP 的家庭 · Route B

  1. 先了解现状:AFA 目前还不是区域中心的签约供应商(vendored)。 服务协调员现在无法用传统方式直接向 AFA 购买服务。我们正在申请签约(服务代码 094 创意艺术课程),审批时间由区域中心决定,不能承诺日期。 / AFA is not yet a vendored provider; we are applying (Service Code 094) but cannot promise a date.
  2. 告诉 SC 您想加入 SDP。 任何符合条件的区域中心服务对象都可自愿参加;SDP 里除 FMS 外的服务提供者不需要是签约供应商。 / Tell your SC you want to enroll in SDP; providers other than the FMS do not need to be vendored.
  3. 参加 SDP 说明会(由 SCDD 统一举办,scdd.ca.gov/sdp-orientation)。 / Attend the SDP orientation run by SCDD.
  4. IPP 团队按过去 12 个月的服务费用确定个人预算,选一家 FMS(必选);独立协调员可选。 / Budget is based on the last 12 months of services; choose an FMS.
  5. 做支出计划,把 AFA 写进去,回到 A 路线第 1 步。 等待期间可先自费上课。 / Build the spending plan with AFA in it, then Route A step 1.

  • 服务协调员的姓名、电话、邮箱 / Service Coordinator contact
  • 如已在 SDP:FMS 名称、独立协调员联系方式、当前支出计划里有没有艺术课 / FMS, IF, whether the plan already has an art line
  • IPP 里相关目标(口头说即可) / Related IPP goals (verbal is fine)
  • 孩子的兴趣和需要注意的地方 / The child's interests and needs

孩子的姓名、UCI 编号和计划内容严格保密,不出现在任何宣传材料里。 / Child's name, UCI and plan details stay confidential.

常见问题 · FAQ

家长要付钱吗? 已批准的课程不用,AFA 直接向 FMS 开票。 / Families pay nothing for approved sessions.
一周几节? 通常每周一节 1.5 小时一对一,画材包含。 / Usually one 1.5-hour session per week.
要先评估吗? 不需要,一次入学沟通即可。 / No formal assessment.
能进普通班吗? 条件合适可进周末常规班。 / Weekend group classes when appropriate.
语言? 中文、英文。 / Mandarin and English.
